APPLICATION CRITERIA
In order to qualify for consideration for accommodation at Trinity Apartments, please note the following:

The scheme is intended for people aged 55 and over with a need for care and support.

You must already live in the Metropolitan Borough of Solihull or have a strong family connection with the area (Note; First priority will be given to applicants who currently live in the Borough).

You must have the ability to live independently with care and support provided.

You must have some personal care and/or support needs and be willing to take part in a community assessment which will identify your level of need.

If you meet these basic eligibility criteria, you are invited to complete an application form for accommodation.

Consideration of a completed application will necessitate a home visit being arranged to view your existing accommodation and to discuss your care and support needs.

A report on your application will then be presented to an Allocations Panel which will decide one of the following:

Your application is approved and you are to receive an offer of accommodation.

Your application is approved and you are to be admitted onto the Trinity Apartments waiting list.

Your application has been unsuccessful.

Whatever the decision, you will be notified accordingly as soon as possible once a decision has been made.


EXTRA CARE SERVICES
A non-resident scheme manager employed by SCHA will oversee the smooth running of the scheme. Duties of the scheme manager will include:
•Making daily contact with all residents and dealing with day-to-day queries
•Ordering of repairs and maintenance to communal areas
•Ensuring the provision of cleaning and gardening services to communal areas to the required standards
•Ensuring meals provided are of the required standard
•Arranging and promoting a regular programme of activities for residents
•Assisting residents with benefit claims and accessing the services they need
•Regular testing of fire and security alarms
•Liaising with residents' families and friends
Following assessment, you will receive a care and/or support plan suited to your individual needs, provided by either the on-site care and support provider commissioned by Solihull Council or a provider of your choice.
If you do not qualify for the cost of care being met in full or in part by Solihull Council, you may seek to purchase it privately either from the on-site care and support team or another provider.
As and when your needs change, your care and support plan will be reassessed.


SUPPORTING PEOPLE WITH A VARIETY OF NEEDS
At Trinity Apartments, we offer accommodation to people with many different needs. Some people have only a low level need for support whilst others may need more support to help them remain in their own apartment.

For example, whilst we are not a specialist service for people with dementia, we are able to offer accommodation to people with early to middle stage dementia with the support of the 24 hour son-site care and support provider and use of the latest assistive technology.

We and the care and support provider also work closely with health, social care and other agencies as well as family carers to ensure that people are able to remain in their own accommodation for as long as possible whilst ensuring that we give an excellent service to all residents, whatever their level of need.

In any circumstances where an individual has health needs which can no longer be met with additional visiting support, we will talk to the individual and those concerned, such as immediate family and help to find a more suitable solution to the individual's housing and care needs.

SHARED OWNERSHIP SERVICE CHARGE
A weekly service charge of £97* is payable by shared ownership purchasers towards meeting the cost of providing services at Trinity Apartments which include:

- Employment of a full-time scheme manager and part-time administrator.
- SCHA Head Office management and support costs.
- Buildings insurance (Note: Purchaser responsible for insuring own contents)
- Day-to-day repairs to communal areas inc fixtures and fittings.
- Redecoration to communal areas
- Grounds maintenance
- Cleaning of communal areas
- Cleaning and replacement of communal area furnishings
- Infrastructure cost for catering
- Electricity to communal areas (Note: Electricity in each flat separately metered and billed direct to purchaser)
- Gas to communal areas and apartment
- Water supply to communal areas and apartment
- Clinical waster
- Communal licences

The service charge is reviewed annually and remains payable to SCHA throughout the period of ownership irrespective of whether or not the apartment is occupied.

OTHER CHARGES PAYABLE TO SCHA
Meal Charge - A weekly charge of £25* is payable for the provision of a two course midday meal (Note: In the event of a resident being unable to order and consume a meal due to either being unwell or away from the scheme due to holiday or illness, the meal charge may be adjusted accordingly.)

Major Repairs and Admin Sale Charge - No contribution towards a sinking fund provision for future major repairs to communal areas is included in the weekly service charge.
